Background Poor experience of care at the end of life (EOL) is often visible through the reporting of incidents and complaints or feedback via the Medical Examiner Service. However these mechanisms in isolation rarely allow for robust thematic evaluation or co-design of potential solutions to improve experience with both bereaved relatives and staff.Methods This study combines an initial deep dive sprint to gather user insights, with a particular focus on the involvement of bereaved relatives from underrepresented groups within the North West London population, followed by a co-design phase to ideate and test a potential service improvement with lived experience representatives and staff.Results Twenty interactions, twelve 60 minute interviews and eight written responses, with lived experience representatives generated ten key findings, three of which were taken forward in combination. These were 1) How we share information about the services, amenities and support on offer, 2) How we empower people to do their best to support and care for loved ones and themselves near the EOL in a hospital environment and 3) How we help support communication with hospital staff. ombined with a high level literature review, the co-design phase led to the conception of the Purple Butterfly Model of Care (PBMoC). This model encompasses the use of a Purple Butterfly symbol and education & training for staff alongside essential resources and co-designed written information, created by bereaved relatives for those soon to be bereaved.Conclusion The systemic engagement with user insights from lived experience representatives has strengthened the development of the PBMoC. The PBMoC has been well received by staff and patients/relatives, and has led to improvement in staff skillsets, and family feedback.
