Of course, that means different things to different people. My family has gathered every year for 75 years at my cousin’s home in Tempe, AZ. Two of the cousins were adopted as children. Since they were born Native Americans, do they have any regrets about how welcoming the first Native Peoples were to those strange strangers from across the ocean? Maybe. I’ve never asked that question. But it clearly did not turn out well, that hospitality, for their ancestors.

Bictegravir/emtricitabine/tenofovir alafenamide (BIC/FTC/TAF) is a highly potent antiretroviral regimen with a high genetic barrier to resistance. Clinical trials have not reported treatment-emergent resistance in treatment-naive individuals. We describe a rare case of virologic failure with an emergent R263K integrase mutation in a treatment-naïve, adherent individual maintained on BIC/FTC/TAF. The patient achieved initial suppression for nearly two years before rebound viremia with R263K and D67N mutations. Following the regimen change and treatment of concurrent syphilis, viral suppression was re-established. This case highlights that emergent bictegravir resistance can occur in adherent, treatment-naïve patients, emphasizing the importance of continued resistance monitoring.

This update is prompted by recent advancements in the diagnosis and treatment of adult CD. The primary goal is to emphasise evidence-based practices to enhance the management of adult CD, with the aim of improving patient care.
