It is quite common for hospitals to hire physicians, particularly emergency department physicians, as independent contractors, rather than employees, in order to reduce the chances for legal liability. A hospital can often avoid liability for actions or inactions of contract physicians that result in lawsuits by claiming that the physicians were not employees and thus the employer cannot be held responsible. A recent case may indicate that this is going to change.

Linsitinib is an oral small molecule that inhibits the activity of insulin-like growth factor-1 receptor (IGF-1R). It is believed that inhibition of IGF-1R may help reduce orbital inflammation and associated tissue damage that occurs in TED patients.

A 53-year-old woman presents for evaluation of recurrent ulcers that develop, heal, and then reappear on her right breast over the past 5 years. The lesions typically start as blisters, ulcerate, and leave healed scars (Figures 1 and 2). Lesions are pruritic with a severity of 8 out of 10, affecting her sleep and daily functions. She denies fever, chills, or any history of skin cancer. A physical exam of the right breast shows pustules and healed ulcerative wounds (Figures 3 and 4). The patient's history reveals that a bilateral mastectomy with an implant was initially performed to treat the infection. Multiple biopsies and wound cultures were collected. She was initially given systemic steroids (ie, methylprednisolone) and antibiotics (ie, doxycycline), which improved symptoms. The symptoms recurred with the withdrawal of treatment. Her condition was also refractory to trials of cyclosporine, dapsone, and upadacitinib.

Blarcamesine is an investigational oral sigma-1 receptor (SIGMAR1) agonist. Activation of SIGMAR1 is expected to restore cellular homeostasis by inducing autophagy, which may slow neurodegeneration in patients with early AD.
