Background There is a paucity of data regarding the comparison of asparaginase activity, pharmacokinetics, pharmacodynamic outcomes, and safety of biosimilar native E.coli asparaginase (N-Asp) and biosimilar pegylated asparaginase (P-Asp) used in India. Methods A randomized, open-labeled, population pharmacokinetic (PopPK) and pharmacodynamic trial comparing the N-Asp (10,000 IU/m2) and P-Asp (1000 IU/m2) administered intramuscularly in children with newly diagnosed acute lymphoblastic leukemia. The primary endpoint was asparaginase activity on day 33 of the induction phase of chemotherapy. Population pharmacokinetic modeling was done after the first dose in both groups, and the models were simulated to assess for the subtherapeutic trough concentrations. Results Fourteen patients were randomized to each arm. The asparaginase activity of 434[interquartile range(IQR) 259, 629] IU/L was observed in the P-Asp compared (p—0.04) to the N-Asp group 64[IQR 27, 184] IU/L on day 33 of the induction phase of chemotherapy. The anti-asparaginase antibody formation, minimal residual disease, and safety were comparable between both groups. The interindividual variability of the population parameters in the PopPK model was higher with N-Asp than P-Asp. The subtherapeutic trough levels of asparaginase were observed more in the N-Asp group (9/14 in N-Asp vs. 0/14 in P-Asp, p < 0.01). Conclusion The asparaginase activity was higher with P-Asp at d33. P-Asp demonstrated less interindividual variability and a predictable kinetic profile, supporting its preferability over N-Asp for first-line management of ALL in the Indian pediatric population. While routine TDM for trough level monitoring may be less critical with P-Asp, TDM retains value to identify silent inactivation, differentiate hypersensitivity from infusion reactions, and confirm adequate activity in high-risk scenarios. In contrast, the higher variability of N-Asp strongly warrants the use of TDM. Clinical trial registration identification CTRI/2019/06/019520 registered on 04/June/2019.

This study examines optical cavity optimization in MAPbI3 perovskite solar cells using one-dimensional photonic-crystal reflectors. Baseline, front-DBR and back-DBR configurations were analysed over 400-850 nm with a numerically stable scattering-matrix formulation based on the Redheffer star product. Absorber-specific perovskite absorptance, angular response up to 70°, field distributions and generation profiles were evaluated so that useful absorption inside MAPbI3 could be separated from total stack absorption. In the idealized optical-stack model, the back DBR increased Jsc from 19.30 to 20.78 mA cm−2, whereas the front DBR reduced Jsc to 12.12 mA cm−2 because of entrance-side reflection losses. A more realistic integrated cavity, Air / ITO / SnO2 / MAPbI3 / NiOx / DBR / Ag / Glass, was then optimized over DBR period number, design wavelength, perovskite thickness, NiOx thickness and rear-metal thickness. The optimized practical design, Air / ITO(150 nm) / SnO2(40 nm) / MAPbI3(800 nm) / NiOx(1 nm) / [TiO2(50 nm) / SiO2(85 nm)]x8 / Ag(60 nm) / Glass, increased the directly simulated optical current from 22.36 to 22.86 mA cm−2. The results show that rear photonic-crystal reflectors can improve long-wavelength harvesting, but in realistic PSCs the gain arises from full-cavity tuning rather than from a standalone quarter-wave DBR design. The DBR material pair was intentionally fixed to the literature TiO2 / SiO2 system so that reflector placement and cavity tuning could be isolated clearly.

This work presents a compact dual-band millimeter wave (mmWave) antenna that operates at 17–27 GHz and 36–43 GHz, with resonance frequencies centred at 21.1 GHz and 38.3 GHz, respectively. The multiple embedded arc-shaped mmWave antenna geometry provides compact, effective dual-band operation. The concentric arcs of the antenna structure are effectively optimized to produce a dual band of operation with good impedance matching. The resulting antenna has gain of 4.8 dBi at 26 GHz and 6.1 dBi at 37 GHz frequencies. The developed antenna is intended for widespread usage in satellite and 5G networks. Despite providing significant bandwidth for 5G and other next-generation communication systems, mmWave frequencies are susceptible to path loss, particularly in congested urban areas. This study further investigates the performance of the designed dual-band mmWave antennas in various urban environments. Both Line-of-Sight (LOS) and Non-Line-of-Sight (NLOS) scenarios are considered to capture realistic deployment conditions. To accurately model propagation behaviour, the analysis incorporates the ABG and CI path-loss models, which comprehensively account for environmental factors such as shadowing, diffraction, and precipitation.

Background Tuberculosis (TB) remains a significant public health challenge in India. Malnutrition and TB form a vicious cycle, where malnutrition predisposes a child to acquire TB, while TB could exacerbate undernutrition. In India, Nutritional Rehabilitation Centers (NRCs) provide therapeutic nutrition and medical care to address malnutrition in children. The National Tuberculosis Elimination Program (NTEP) was integrated into India’s NRCs in 2012 for early detection of TB among malnourished children; however, its implementation remains suboptimal. The present study aims to identify the challenges and barriers in implementing NTEP guidelines at NRCs in India through systematic review of literature. Methods The systematic review was conducted across six databases including PubMed, Scopus, ScienceDirect, WHO Global Index Medicus, Semantic Scholar, and Google Scholar for studies published between 2010 and 2024. The systematic review protocol has been registered in the PROSPERO web portal with Registration ID No. CRD42025644172. The quality of individual studies included in the evidence synthesis was assessed using the Mixed Methods Appraisal Tool (MMAT) - Version 2018. A narrative synthesis approach, following the SWiM (Synthesis Without Meta analysis) guidelines, was employed for data analysis. Results Out of 1,374 records identified through database searches, four studies met the inclusion criteria and were included in the final synthesis. Key challenges and barriers reported in implementation of TB screening and diagnosis were (1) underutilization of diagnostics, (2) human resource constraints, (3) sample collection issues, (4) treatment gaps, and (5) infrastructural limitations. Addressing these barriers with context-specific innovative strategies is essential to improve TB screening and diagnosis among malnourished children admitted in NRCs. Conclusion This systematic review highlights that while NRCs play a crucial role in managing SAM, the underutilization of diagnostic tools, limitations in human resources, difficulties in sample collection, and infrastructural and logistical barriers have resulted in the potential underdiagnosis of TB in this vulnerable population. This systematic review provides valuable insights into the issues that must be addressed to improve TB diagnosis among SAM children admitted at NRCs in India.
